Personal Support Worker (PSW)

Job Description

Hire Horizon Inc is currently seeking compassionate and dedicated Personal Support Workers (PSWs) to support our client facilities, including long-term care homes and healthcare settings. The successful candidates will provide client-focused personal care in a respectful, safe, and supportive environment while working as part of an interdisciplinary care team. This role is ideal for professionals who are passionate about improving the quality of life for residents and patients.

Availability Requirements

• Ability to work weekday shifts (7:00 AM – 3:00 PM and/or 3:00 PM – 11:00 PM)
• Availability to work every other weekend (Saturday and Sunday)
• Hours are not guaranteed and will vary based on client needs

Qualifications

• Completion of a PSW Certificate from a recognized college
• Active registration on the PSW Registry
• Previous experience in a long-term care or healthcare setting is an asset
• Vulnerable Sector Police Clearance
• Completed TB Test (within 6 weeks of job offer)
• Strong communication skills, professionalism, and reliability

Key Responsibilities

• Provide personal care including bathing, hygiene, grooming, toileting, and skin care
• Assist residents with mobility, transfers, and range-of-motion exercises using mechanical or ceiling lifts
• Document care activities using Point of Care (POC) systems
• Assist with meals, feeding, intake/output recording, and basic non-prescription care as directed
• Monitor and report any changes in residents’ physical, cognitive, or emotional condition to RN/RPN staff
• Prepare residents for medical appointments, activities, and services
• Support residents’ emotional, social, and spiritual well-being and encourage independence
• Assist with admissions, transfers, and discharges
• Maintain cleanliness and safety of resident areas and equipment

Health, Safety & Compliance

• Follow all health and safety, infection control, and fire safety policies
• Use appropriate personal protective equipment (PPE) at all times
• Report incidents, hazards, and safety concerns promptly
• Complete timesheets, incident reports, and attend required meetings and training sessions
• Work collaboratively with team members and contribute to a positive care environment
